Hamdoroid from Hamdard is proprietary Unani medicine. It is indicated in piles and available in the form of Hamdoroid capsule for internal use and Hamdoroid Ointment for topical use.

Piles are also known as Bawaseer and Haemorrhoids. It is the most prevalent anorectal disorder and the most common cause of bleeding with stool. Patients with a first degree or second degree, Haemorrhoids may experience acute attacks of discomfort, pain and/or bleeding. Rasanjana or Rasont or Rasaut

Daruharidra or Berberis Aristata is a shrub or small tree. It is distributed in the temperate and subtropical parts of Asia, Europe, and America. Barberry, Berberidis, Pipperidge bush. Berberis chitria Lindl., Berberis dumetorum Gouan, Berberis floribunda Wall. are few common names of Berberis Aristata.

Properties of Daruhaldi is considered similar to Haldi. Daruhaldi is effective against a variety of bacteria, fungi, protozoa, helminths, chlamydia, and viruses.

The main constituent berberine is bactericidal, amoebicidal and fungicidal. It has some antiepileptic, uterine stimulant and hypotensive effects and is slightly sedative. The decoction of the roots and bark is given for jaundice, diarrhea, malarial fever and painful micturition due to bilious or acrid urine. Externally, it is used in all ailments of the eye, skin diseases and for cleaning ulcers.

Rasanjana or Rasont is the solidified decoction of Daruhaldi. The decoction or solidified water extract is prepared from the stem or the roots of the plant. Rasont has anti-inflammatory, antimicrobial, anti-diarrheal, anti-trachoma and antipyretic activity.

Rasont is Uterine stimulants. Uterine stimulants(uterotonics) are medications that cause the uterus to contract or increase the frequency and intensity of the contractions.

Guggulu

It is gum resin of a tree Commiphora Mukul, found in arid- subtropical to tropical climate. Guggulu is highly effective in the treatment of Vata-vikar and inflammatory conditions. The gum resin contains guggul sterons Z and E, guggul sterols I-V, two diterpenoids- a terpene hydrocarbon named cembreneA and diterpene alcohol- mukulol, a-camphrone, and cembrene, long chain aliphatic tetrols- octadecan-1,2,3,4-tetrol, eicosan-1,2,3,4-tetrol, and nonadecan-1,2,3,4-tetrol. Major components from the essential oil of gum resin are myrcene and dimyrcene.

The gum resin is bitter, acrid, astringent, thermogenic, aromatic, expectorant, digestive, anthelmintic, anti-inflammatory, anodyne, antiseptic, demulcent, carminative, emmenagogue, haematinic, diuretic, rejuvenating and general tonic.

It is effective in reducing the synthesis of cholesterol in the liver. It reduces serum triglycerides, LDL (bad) cholesterol and increases HDL. It can reduce 14-27 percent blood cholesterol and triglycerides by 22-30 percent. Guggulu increases metabolism of LDL.

Guggulu is used in Ayurveda for the treatment of arthritis, rheumatism, gout, lumbago, disorders of the nervous, respiratory, digestive and circulatory systems.

It has anti-inflammatory with strong purifying and rejuvenating properties.

Neem

Neem (Azadirachta indica, family Meliaceae) is also known as margosa, nim, Nimba, nimbatiktam, arishtha, praneem. The active constituents of neem are the mainly triterpenoids, sterols, bitter principles Nimbin and nimbiol. The Neem bark is cool, bitter, astringent, acrid and refrigerant. The bark of the tree is useful in tiredness, cough, fever, loss of appetite, worm infestation.

Neem bark heals wounds and vitiated conditions of kapha, vomiting, skin diseases, excessive thirst, and diabetes. Neem is used to treat intestinal worms, heat-rash, boils, wounds, jaundice, leprosy, skin disorders, stomach ulcers, chicken pox, malarial fever, etc. The bark of the tree is used in Ayurveda for the treatment of the skin diseases, excessive pitta, excessive kapha, toxins in the body, bleeding disorders and healing of wounds.

Neem Root bark and young fruits are astringent, tonic and antiperiodic (prevent recurrence of diseases).

Hamdoroid capsule and Hamdoroid ointment seem to have a therapeutic role in producing relaxation of the anal sphincter and reduction in inflammation and edema. Acute attacks (grade 3 severity of pain and/or bleeding) are relieved significantly with this combination.

The reduction in inflammation and pain is due to the local effects of the analgesic property of Kafoor(Cinnamomum camphora), and anti-inflammatory and healing promoting properties of Rasaut (Berberis aristata), Roghan Neem (Azadirachta indica), and Ritha (Sapindus mukoressi).

Application of topical Hamdoroid is beneficial in relieving pain because of sedative and anesthetic properties of its ingredients Kafoor (Cinnamomum camphora), Sat-e-Pudina (Menth apiperita) Safeda Kashghari (Plumbi Carbonate) and Roghan-e-Arand (Ricinus communis Linn.). Kafoor (Cinnamomum camphora), Sat-e-Pudina (Mentha piperita), Maazu (Quercus infectoria) and Roghane-Neem (Azadirachta indica) also possess antiseptic properties.

Bleeding is arrested because of the Hemostatic property of Maazu (Quercus infectoria) and Safeda Kashghari (Plumbi Carbonate) and astringent activities of Rasaut (Berberis aristata) and Suhaga (Sodii Biboras).

It causes shrinkage of pile masses and lessening of prolapse.

Suggestions

Piles or Hemorrhoids are abnormally enlarged anal cushions. They are swollen and inflamed veins around the anus or in the lower rectum. External hemorrhoids are located under the skin around the anus. Internal hemorrhoids develop in the lower rectum. Internal hemorrhoids may protrude, or prolapse, through the anus.

Most prolapsed hemorrhoids shrink back inside the rectum on their own. Severely prolapsed hemorrhoids may protrude permanently and require treatment.

Excessive straining, rubbing, or cleaning around the anus may make symptoms, such as itching and irritation, worse.

Hemorrhoids are not dangerous or life-threatening. Symptoms usually go away within a few days, and some people with hemorrhoids never have symptoms.

The single most important management of piles is to increase the daily fiber intake to above 25 g/day by the diet with/without fiber supplements.
